These Terms of Service (“Terms”) govern your use of the services provided by Vantage Verify, LLC (“Company,” “we,” “our,” or “us”), including mobile and in‑office DNA collection, drug and alcohol testing, fingerprinting, notary services, and background screening. By scheduling an appointment, accessing our website, or using any of our services, you agree to these Terms.
If you do not agree to these Terms, you may not use our services.

1. Services Provided
Vantage Verify, LLC provides the following services:
Mobile DNA collection
Drug and alcohol testing
Fingerprinting
Notary services
Background screening (CRA)
Identity verification
Compliance documentation
All services are performed according to industry standards, legal requirements, and chain‑of‑custody procedures where applicable.

2. Eligibility for Services
You must be:
At least 18 years old, or
A parent/legal guardian of a minor, or
An authorized representative of an employer, attorney, or agency
We may request identification or documentation to verify eligibility.

3. Appointment Requirements
3.1 Identification
All clients must present valid government‑issued identification. Failure to provide ID may result in cancellation or refusal of service.
3.2 Attendance
Only the individual being tested may enter the testing area unless legally required (e.g., minor with guardian).
3.3 Late Arrivals
Arrivals more than 20 minutes late may require rescheduling and may incur a Late Arrival Fee.
3.4 No‑Shows
Failure to appear within 20 minutes of the scheduled appointment time is considered a No‑Show and may incur a fee.
3.5 Refusal to Comply
Any refusal to follow testing procedures, identification requirements, or attendance rules may result in cancellation and may be treated as a No‑Show.

4. Mobile Service Requirements
For mobile appointments:
The location must be safe, accessible, and appropriate for testing.
The client or organization must ensure individuals are present and prepared.
Travel fees may apply depending on distance and service type.
We reserve the right to decline mobile service if conditions are unsafe or unsuitable.

5. Payment Terms
5.1 Payment Due
Payment is due at the time of service unless otherwise arranged with an employer, attorney, or agency.
5.2 Accepted Payment Methods
We accept:
Credit/debit cards
Cash
Employer/agency billing (approved accounts only)
5.3 Refunds
Refunds are not provided for:
No‑Shows
Late arrivals
Refusal to comply
Completed services
Refunds may be considered only if Vantage Verify cancels a service due to internal error.

6. Background Screening (CRA)
When providing background screening services, Vantage Verify operates as a Consumer Reporting Agency (CRA) under the Fair Credit Reporting Act (FCRA).
Employers must:
Provide proper disclosures
Obtain written authorization
Follow adverse action procedures
Use reports only for permissible purposes
Individuals have rights under the FCRA, including the right to dispute inaccurate information.

Clients, employers, and agencies are responsible for providing accurate information. We are not liable for delays or errors caused by incorrect or incomplete information.

Vantage Verify is not liable for indirect, incidental, or consequential damages.
Our liability for any claim is limited to the amount paid for the specific service.
We are not responsible for decisions made by employers, courts, agencies, or laboratories based on test results or reports.

10. Service Refusal
We reserve the right to refuse service to any individual who:
Provides false information
Refuses to comply with procedures
Behaves in a threatening or unsafe manner
Violates these Terms

11. Changes to Terms
We may update these Terms at any time. Updates will be posted on our website with a revised effective date.
Continued use of our services constitutes acceptance of updated Terms.

These Terms are governed by the laws of the State of Tennessee. Any disputes shall be resolved in the appropriate courts located in Shelby County, Tennessee.
